Martha’s Vineyard Regional High School
Wednesday, NOVEMBER 11, 2009,
8am to 12 noon
WHO SHOULD COME?
Pregnant women **  People 50 years of age and older **  Adults with certain chronic medical conditions  ** Health Care workers, EMTs, and People who live with or care for those with chronic medical conditions ** This clinic will only vaccinate adults 18 years or older.  All others please see your doctor to receive the vaccine.
HOW TO PARTICIPATE:
We encourage you to pick up a Registration Form and access/staging information in advance of the clinic at your local Town Hall, Council on Aging, Library, Pharmacy or visit the Dukes County Website (www.dukescounty.org).  


 On November 11th, vehicle traffic MUST register at WABAN PARK in Oak Bluffs, or the MV AGRICULTURAL HALL in West Tisbury. After registration, you will be directed to the High School to get vaccinated.

REMEMBER TO:

·       Wear a short-sleeved shirt for easier access to your arm
·       Bring your registration form.
·       Bring your Medicare or Insurance information You do not need medical insurance to qualify.  The vaccination is free of charge.

*Please note: THIS IS NOT THE SWINE FLU VACCINE   This vaccination will help protect you against this year’s seasonal flu.  It will not protect you from the 2009 H1N1 “Swine Flu”.  Vaccination with 2009 H1N1 vaccine will be done when the vaccine is available.

All-Island Seasonal Flu Immunization Clinic
Wednesday, November 11th 2009

IMPORTANT INFORMATION:  There will be two vehicle staging areas for the seasonal flu clinic, Waban Park in Oak Bluffs and the Ag Hall in West Tisbury.  On the day of the clinic anyone driving to the clinic must register at one of the staging areas prior to being directed to the high school for your vaccine.  VEHICLES SHOULD NOT REPORT DIRECTLY TO THE HIGH SCHOOL.  Vehicles reporting directly to the high school will be turned away and advised to go to one of the staging areas to register.  Walkers, bicycle riders and those arriving by VTA should go directly to the high school to check-in.

This is the only TOWN-SPONSORED seasonal flu clinic scheduled this year. ~

You should attend this clinic if you: are 50 years of age or older; an adult under age 50 with a chronic medical condition being treated by a physician; will be pregnant during the flu season; or if you are a health care worker or a first responder with direct patient contact

This clinic will serve only adults 18 years or older.  Anyone under the age of 18 should consult his/her Primary Care Physician regarding seasonal flu vaccine.

This clinic is for seasonal flu vaccine, NOT H1N1 “Swine Flu” vaccine

Please complete the attached forms and bring them with you to the Flu Clinic
General Directions:
w       Fill out Parts 1 & 2 on the front of the form. ~

w       Medicare Part B covers the cost of these vaccines and will be billed for this service. ~Fill in your Medicare number on the form. ~Remember to bring your Medicare card with you to the Clinic.

w       If you do not have Medicare Part B you will still be immunized free of charge

w       Complete the Screening Questionnaire on the back of the form

w       Complete the top portion of the attached Tier Group Screening form

w       We will provide your island doctor or clinic a copy of this form as official documentation of the vaccine given to you.
w       Wear a short-sleeved shirt to the clinic.
