All-Island Flu Immunization Clinic
Monday, November 12th 2007
9 am – 1 pm
Please arrive no later than 12:30 pm to assure timely processing
at the
Martha’s Vineyard Regional High School Gymnasium
This is the only TOWN-SPONSORED clinic being held this year. ~
You should attend this clinic if you: are 50 years of age or older; have a chronic medical condition currently being treated by a physician; will be pregnant during the flu season; or if you are a health care worker or a first responder with direct patient contact
Please complete the attached form and bring it with you to the Flu Clinic
General Directions:
w Fill out Parts 1 & 2 on the front of the form. ~
w Medicare Part B covers the cost of these vaccines and will be billed for this service. ~Fill in your Medicare number on the form. ~Remember to bring your Medicare card with you to the Clinic.
w If you have private health insurance provide that information and we will bill them if possible. Remember to bring your insurance card with you to the Clinic.*
w If you do not have any health insurance you will still be immunized.*
w Complete the Screening Questionnaire on the back of the form
w We will provide your island doctor or clinic a copy of this form as official documentation of the vaccine given to you.
w Wear a short-sleeved shirt to the clinic.
Parking & Transportation:

Those with mobility concerns will be directed to park in back of the High School.
Everyone else will park and enter at the Performing Arts Center.
